条款与细则
  • 根据马来西亚政府规定，非马来西亚公民购买健康体检套餐和康复产品需缴纳6%的销售和服务税（SST）。可能还需支付额外附加费。
  • 可先购买，后预约筛查。
  • 配套需在购买之日起3个月内预约使用，逾期作废。
  • 所有购买行为一经确认，恕不退款。
  • 此配套仅适用于 18 岁及以上成人。
Bank Promotion Terms & Conditions
  1. Payment must be made with a Maybank Visa, Mastercard (issued by Maybank, Maybank Islamic Berhad, Maybank Singapore Limited, PT Bank Maybank Indonesia, Maybank Philippines Inc., Maybank Cambodia Plc. or Maybank Brunei Darussalam, as the case may be), American Express (issued by Maybank and Maybank Islamic Berhad) & JCB card (issued by PT Bank Maybank Indonesia).
  2. Offers cannot be exchanged for cash or used in conjunction with any other promotions, offers, loyalty card or coupons.
  3. The discount is only applicable to non-promotional items.
  4. The discount is not applicable to existing on-going scheme with corporate clients.
  5. Customers will need to make prior appointment with SMCV Health Screening Centre, failing which no discount will be granted.
  6. The discount will only be given if the total bill has been paid in full by the customer before the customer is charged from or leaves SMCV.
  7. Offer is applicable to charges for the screening package(s) only, excluding any other additional charges and/or relevant taxes including SST for international patients who does not hold a MyKad.
  8. Offer is subject to other terms and conditions imposed by SMCV

Purchase & Booking Mechanism for 10% Maybank

  1. Maybank cardholders may walk-in or visit https://shop.sunwaymedicalvelocity.com.my/ for package purchase purpose. Redemption must be done within 3 months from date of purchase.
  2. All appointments must be done via SMCV Health Screening Centre website: https://smcv.sunmedgo.com/#selectDateTime/8. Alternatively, appointment can also be secured via walk-in, telephone call to +603-9772 9229/9191 or email to [email protected] with the quote “Maybank Wellness Program” in the email.
  3. All bookings must be accompanied by personal details for registration purpose. During the registration at the SMCV HSC counter, patients/customers are required to present their Maybank Visa, Mastercard (issued by Maybank, Maybank Islamic Berhad, Maybank Singapore Limited, PT Bank Maybank Indonesia, Maybank Philippines Inc., Maybank Cambodia Plc. or Maybank Brunei Darussalam, as the case may be), American Express (issued by Maybank and Maybank Islamic Berhad) & JCB card (issued by PT Bank Maybank Indonesia).
